All middle school and high school students who wish to participate in activities (athletics, fine arts, speech, drama, clubs, etc.) are required to register online and have a signed physical form on file prior to beginning practices. Please be sure to complete both pages of the physical form. In addition to a physician's signature, a parent/guardian signature will also be required.
